To apply for any area of accreditation please fill in the associated application form.
Please also include the following documentation
- A copy of your passport
- A copy of your driving licence
- Your National Insurance number
- A copy of your relevant qualification diploma
- A copy of your complaints procedure (please see accreditation resources for document templates if required)
- If you are applying for domestic accreditation please also include a copy of your CRB document (basic) - these can be requested from Disclosure Scotland (Click Here)

When you have filled in the application form please email it to applications@quidos.co.ukalong with any additional requested information.
Upon successful submission, we will e-mail a Membership Agreement form for you to sign and return.

How to pay:
Pay by telephone:
For payments by phone, please call the accounts department on 01225 318 400 (option 4) to pay direct.
By Cheque:
Please send a cheque for the amount requested to the following address: 10 Argyle street, Bath, BA2 4BQ.
By Direct Debit:
Please download the Direct Debit mandate form here. You will be required to send the mandate form to us along with either a void cheque or a bank statement with the transactions blanked out, which will enable us to verify the account details. Please post all forms and supporting documents to:
