To apply for any area of accreditation please fill in the associated application form.
Please also include the following documentation:
- A copy of your passport
- A copy of your driving licence
- Your National Insurance number
- A copy of your relevant qualification diploma
- A copy of your complaints procedure
- If you are applying for domestic accreditation please also include a copy of your CRB document - which is available from Disclosure Scotland (Click Here)
Please download your application form and code of conduct below.
When you have filled in the application form simply scan it in together with the associated documentation and email it to applications@quidos.co.uk or post it to unit 7, Handlemaker road, Frome, Somerset BA11 4RW.
Once we have processed you application we will send out a Membership Agreement from in the post for you to sign.
